Costco Mail Order - New Patient Instructions
This tutorial will aid new mail-order customers in creating their account and submitting their first prescription order.
Create an account
The instructions below will walk you through setting up an account for Costco Mail Order.
Instruction List
- On the Costco website, click on Pharmacy in the menu.
- Then click on Rx Mail Order Visit Mail Order.

- Click on Fill New Prescriptions.

- Click on Create Account
Note: If you already have a Costco account Sign in with your login information instead. Each patient is required to create an individual account.

- Enter in the information requested on the Create Account Form.
Note: Membership Number is optional.

- Click on Complete Patient Profile.

- Complete required patient information.

- Set your profile preferences.
- Click Next

- Please provide your insurance information.

- Enter your Cardholder Information.

- Enter your Payment Information.

- Add a new shipping address.
Costco Mail Order uses the same addresses on file for Costco.com. Please verify the correct shipping address.

- Add address information.

- Add a new billing address
If the shipping and billing address are the same, select add new address and check the Same as Shipping Address checkbox

- Read the privacy acknowledgement
- Check the "I have reviewed..." Box
- Select Complete Profile button
