Important Enrollment Dates

  • Annual Enrollment Period (AEP): October 15 to December 7. During this period, you can enroll in a new plan or make changes to your existing plan.
  • Medicare Advantage Open Enrollment (OEP): January 1 to March 31. You can disenroll from a Medicare Advantage plan to return to Original Medicare or enroll into a different Medicare Advantage plan.
  • Special Election Periods (SEP): Available during the year based on certain events. Call our Member Services Team for more information.

How to Enroll in SECUR HMO I-SNP

Enrolling in a SECUR HMO I-SNP plan is easy and can be done in a few simple steps:

  1. Check Eligibility: Ensure that you meet the eligibility requirements for our Institutional Special Needs Plans (I-SNP). Our plans are available to individuals who are enrolled in Medicare A and B and reside either in a facility or live in the community but require a high level of care within the five Florida counties we serve: Citrus, Hernando, Hillsborough, Pasco, and Pinellas.
  2. Review Your Options: Compare the different plan options available to find the one that best meets your healthcare needs and budget. Visit our Plans and Benefits page to learn more about each option.
  3. Complete the Enrollment Form: You can enroll by completing an enrollment form. Download the SECUR HMO I-SNP Enrollment Form (PDF) and fill it out with your information. If your legal representative is filling out the enrollment form on your behalf, we will need the Attestation of Legal Representative Form (AOR) filled out and accompany your enrollment form.
  4. Fax or Mail Your Form: If you prefer to fax your completed enrollment form, please do so to the following number: 813.357.5352. Or if you prefer to mail us your completed enrollment form, please send to: SECUR Health Plan, 4200 W. Cypress Street, Suite 690, Tampa, FL 33607. We’ll process your application promptly and confirm your enrollment.
