These plans provide comprehensive coverage, including hospital care, doctor visits, prescription drugs, and additional services tailored to the needs of our members.

Who is eligible to enroll in a SECUR HMO I-SNP plan?

To be eligible for a SECUR HMO I-SNP plan, you must be enrolled in Medicare Part A and Part B, reside in the service area of the plan, and meet specific criteria for institutional-level care. This includes individuals living in a facility or those who need equivalent care at home. For more details, contact our Member Services team at 1.833.76SECUR (1.833.767.3287) or TTY 711 who are available 7 days a week from 8 a.m. to 8 p.m. (ET) from October 1 – March 31, and 8 a.m. to 8 p.m. (ET), Monday through Friday from April 1 – September 30.

Can I switch to a SECUR HMO I-SNP plan from another Medicare plan?

Yes, you can switch to a SECUR HMO I-SNP plan during the Medicare Annual Enrollment Period (AEP) from October 15 to December 7, Medicare Advantage Open Enrollment (OEP)from January 1 to March 31. or if you qualify for a Special Enrollment Period (SEP). If you’re already enrolled in a Medicare plan and meet the eligibility requirements for an I-SNP plan, contact us to discuss your options and learn more about making the switch.
