SECUR Advantage (HMO I-SNP)

This plan is available to anyone with Medicare who meets the skilled nursing facility (SNF) level of care and resides in a nursing home in the following counties: Citrus, Hernando, Hillsborough, Pasco, and Pinellas.

Over-the-Counter (OTC) Allowance: Receive a quarterly OTC allowance of $200 to use. Unused funds do not roll over to the next quarter.
Transportation: You receive up to 40 one-way trips per year to plan-approved locations.
Dental Allowance: You receive a $4,500 benefit allowance every year for preventive and comprehensive dental services combined.
Eyewear Benefit: You receive a $360 benefit allowance toward eyeglasses (lenses and frames), upgrades, or a pair of contacts every two years.
Hearing Aid Benefit: You receive a $3,600 benefit allowance toward hearing aids with both ears combined every two years.

SECUR Enhanced (HMO I-SNP)

For those residing in the community in the following counties: Citrus, Hernando, Hillsborough, Pasco, and Pinellas.

Over-the-Counter (OTC) Allowance: Receive a quarterly OTC allowance of $220 to use. Unused funds do not roll over to the next quarter.
Transportation: You receive up to 60 one-way trips per year to plan-approved locations.
Dental Allowance: You receive a $2,550 benefit allowance every year for preventive and comprehensive dental services combined.
Eyewear Benefit: You receive a $250 benefit allowance toward eyeglasses (lenses and frames), upgrades, or a pair of contacts every two years.
Hearing Aid Benefit: You receive a $2,000 benefit allowance toward hearing aids with both ears combined every two years.
Health Food Allowance: Members with a qualifying chronic condition receive $35 per month to use toward health foods and produce. The benefits mentioned are a part of a special supplemental program for the chronically ill. Not all members qualify.
In-Home Services: No cost in-home safety assessment, unlimited bathroom safety devices and you pay $0 for 30 hours of in-home support services per year.

Ready to Enroll?

You may qualify today if you are:

Living in Citrus, Hernando, Hillsborough, Pasco, or Pinellas County.
Turning or recently turned 65.
Be enrolled in both Medicare Parts A and B.
Reside in a facility (SECUR Advantage) or in the community (SECUR Enhanced).

For both plans:

$0 Copays: You pay $0 for primary care visits. Low copay for specialists and inpatient hospital stays.
Dental: Preventive and comprehensive dental services include deep cleanings, fillings, crowns, root canals, extractions, and dentures.
Hearing: Routine hearing exam every year and fitting and evaluation for hearing aids every two years.
Rehabilitation Services: You pay $0 for physical therapy, speech therapy and occupational therapy visits.
